You’ve just taken the next step on your fertility journey with Texas Fertility Center, and we’re so excited.

Now that we have your forms, here are a few things to keep in mind:

• If you have a partner, be sure to remind them to complete their forms before your visit. This will give your doctor the background information they need.
• If you can’t make it to your first appointment, no worries. Just cancel or reschedule at least two business-days before your visit to free up the spot for someone else.
