Two easy steps to schedule a semen analysis at Texas Fertility Center

STEP 1: Requisition form completion

• Physician referral – If you are using a physician outside of TFC, your doctor can print a copy of the order form below or by having their office call 512-610-7474 to receive fax copy. Your doctor does not need to send in the requisition form prior to your arrival.  You may bring this form with you to your scheduled appointment.
• TFC referral – if you are seeing a TFC physician, our staff will send your requisition form to either Ovation Fertility Austin, Ovation Fertility San Antonio or Ovation Fertility New Braunfels.

STEP 2: Schedule an appointment with the lab of your choice in five convenient locations.

• Andrology services are provided by Ovation Fertility Austin, Ovation Fertility San Antonio and Ovation Fertility New Braunfels.
• Lab phones are answered Monday through Friday, from 8:30am to 4:00pm.*
• Due to the sensitive nature of semen samples, you MUST have an appointment in order for our andrologists to promptly process your sample.
• Ovation does not see patients without an appointment.
